What is cardiac rehabilitation?

Cardiac rehabilitation is a multi-dimensional program designed for individuals who suffer from heart disease. It is medically supervised and focuses on the physiological as well as the psychological aspects of recovery.

The goals of the program are to:
Introduce risk factor modification techniques that prevent the progression of development of heart disease.
Increase functional capacity through exercise by developing cardiovascular endurance, strength, and flexibility.
Instill confidence to resume normal activities of daily living.

This flexible, individualized program offers:

 Cardiac Rehabilitation at The Hermann Wellness Center

At Hermann Hospital, Cardiac Rehabilitation is a division of the Hermann Heart Center and consists of four distinct programs.

PHASE I begins as soon as an individual enters the hospital and a cardiovascular event has been identified. This program assists in the process of teaching the patient and family about cardiac disease, how to modify risk factors, and return to a maximum level of functioning upon leaving the hospital. Included in Phase 1 cardiac rehab is telemtery monitored exercise, nutrition counseling, stress management consultations, smoking awareness as needed, and cardiac education.

PHASE II is a 12 week/36 session program initiated immediately after discharge from the hospital. This out-patient program assists the patient and family in adjusting to heart disease and modifying lifestyle. It includes telemetry monitored exercise, nutrition and stress management consultations, smoking awareness classes and CPR training. The participant must have a physician's order for admission.

Treadmill stress tests are performed prior to admission and upon completion to assist in developing a comprehensive exercise prescription. Individuals eligible include, but are not limited to, those who have suffered a myocardial infarction, undergone coronary artery bypass grafting, have evidence of coronary artery disease, and/or have stable angina.

T.R.I.P. (Therapeutic Rehabilitation Intensive Program): This highly specialized program was designed specifically at Hermann for individuals who need Phase II cardiac rehab but do not live in the Houston area. Sessions are attended every day for five or more days and all consult services are provided. If needed, an exercise prescription will be given for a home exercise program.

PHASE III is a maintenance program that follows Phase II. It enables the participant to continue lifestyle modification through non-telemetry monitored exercise and abbreviated versions of nutritional, stress management and smoking awareness counseling. This program may also be utilized by individuals displaying a high risk of cardiovascular disease.

WELLNESS is a program for those individuals wanting a structured environment with a highly trained staff to assist in their exercise program.

 Staffing
The cardiac rehab team consists of a comprehensive health care team trained to work with cardiovascular patients.

THE MEDICAL DIRECTOR, a cardiologist, supervises the clinical management of the patients enrolled in the program. The referring physician remains the primary care provider and is contacted with any problems or concerns regarding patient care.. Reports generated are sent every 12 sessions to the referring physician regarding program progress.

REGISTERED NURSES provide daily clinical management and are an integral part of the cardiac education program.

EXERCISE SPECIALISTS direct the patient in following an exercise prescription tailored especially for them. Home exercise is encouraged as well as monitored.

REGISTERED DIETICIANS consult with the patients at regular intervals during the program and evaluate eating habits and modifications of diet as necessary.

STRESS MANAGERS consult with patients throughout the program assisting in the identification of stressors and method for stress reduction.

 Reimbursement
Most patient insurance companies cover Phases I and II, dependent on the indicidual policy. Medicare covers both Phases as well as entrance and exit stress tests. Phase III and Wellness are monthly charges, usually not covered by insurance.
