Fletcher Factor Assay

SPECIMEN REQUIREMENT: 5 mL blood (minimum 4.5 ml), blue top tube (sodium citrate), mixed well. Test result would not be diagnostic if patient has been on anticoagulant (Heparin or Coumadin) or there is Heparin in sample.

TEST AVAILABILITY: Performed by Coagulation Section in Hermann Laboratory, Mon-Fri, 7 AM-3 PM. Reporting time in 24 hours. Specimen must be in the laboratory by 10 AM to be performed on the same day

METHODOLOGY: Clot-based testing using PTT

CLINICAL UTILIZATION: Diagnosis of Fletcher factor deficiency

REFERENCE RANGE: 60-140 %


INTERPRETATION:

Fletcher factor is one of the coagulation proteins involved in the generation of Bradykinin, and is one of the major factors required for contact activation. The others are Hageman factor (F XII) and High Molecular Weight Kininogen (Fitzgerald factor).

Deficiency of Fletcher factor is associated with a prolonged PTT. Deficiency is inherited as an autosomal recessive trait and does not result in bleeding tendency. Some patients may have thromboembolic episodes. Deficiency of Fletcher factor is a rare disease which should only be considered when a prolonged PTT is not explained by other more common etiologies such as lupus anticoagulant, hemophilia, von Willebrand's disease, Heparin in patient's sample, etc.
